What is EDI in Healthcare?

Electronic Data Interchange (EDI) is the electronic exchange of standardized business documents between healthcare organizations. In the U.S. healthcare system, EDI enables payers, providers, clearinghouses, and other entities to exchange claims, eligibility inquiries, payment information, and enrollment data in a structured, machine-readable format.

HIPAA and EDI Standards

The Health Insurance Portability and Accountability Act (HIPAA) of 1996 mandated the use of standardized electronic transaction formats for all covered entities. The designated standard is ASC X12 (Accredited Standards Committee X12), specifically the X12N healthcare transaction sets.

HIPAA-Mandated Transaction Sets

TransactionX12 CodePurpose
Eligibility Inquiry270Provider asks payer about member coverage
Eligibility Response271Payer responds with coverage details
Claim Status Inquiry276Provider asks payer about claim status
Claim Status Response277Payer responds with claim status
Prior Authorization278Provider requests pre-approval for services
Enrollment834Employer sends enrollment/disenrollment data to payer
Payment/Remittance835Payer sends payment details to provider
Healthcare Claims837Provider submits claims to payer

Key Concepts

Trading Partners

Any two entities that exchange EDI transactions. Common pairs include provider-to-payer, employer-to-payer, and payer-to-payer.

Clearinghouses

Third-party intermediaries that receive EDI transactions from providers, validate formatting, and route claims to the correct payer. Major clearinghouses include Change Healthcare, Availity, Trizetto (Cognizant), and Waystar. Clearinghouses normalize data across different practice management systems and payer requirements.

Code Sets

HIPAA mandates specific code sets for clinical and administrative data:

  • ICD-10-CM/PCS — Diagnosis and inpatient procedure codes
  • CPT (Current Procedural Terminology) — Outpatient procedure codes
  • HCPCS — Healthcare Common Procedure Coding System (includes CPT + additional codes for DME, drugs, supplies)
  • NDC — National Drug Codes for pharmacy claims
  • Place of Service codes — Where the service was rendered

Current State

  • Billions of EDI transactions are processed annually in the U.S.
  • EDI remains the primary mechanism for claims submission, eligibility verification, and payment
  • However, the industry is gradually transitioning to FHIR-based APIs for real-time interoperability, driven by CMS mandates
  • EDI and FHIR will coexist for the foreseeable future
