What are Healthcare Providers?
A healthcare provider is any licensed individual or organization that delivers medical care, diagnostic services, therapeutic treatments, or related health services. In the U.S., every provider that bills Medicare, Medicaid, or private insurance must have a National Provider Identifier (NPI) — a unique 10-digit number maintained in the NPPES registry.
Types of Providers
Physicians
- Primary Care Physicians (PCPs) — Family medicine, internal medicine, pediatrics, and general practice. PCPs serve as the entry point for most patients and coordinate referrals to specialists.
- Specialists — Cardiologists, orthopedic surgeons, dermatologists, oncologists, neurologists, and hundreds of other specialties. Many managed care plans require PCP referrals before seeing a specialist.
Hospitals
- Acute care hospitals — General medical and surgical hospitals providing inpatient, outpatient, and emergency services
- Critical access hospitals (CAHs) — Small rural hospitals (25 beds or fewer) with special CMS designation
- Academic medical centers — Teaching hospitals affiliated with medical schools
- Specialty hospitals — Cardiac, orthopedic, psychiatric, rehabilitation, and long-term acute care (LTAC) hospitals
- Children's hospitals — Facilities focused on pediatric care
Health Systems
Integrated delivery networks (IDNs) that own or operate multiple hospitals, physician practices, and ancillary services under a unified organizational structure.

Provider Identification
All providers are identified through standardized systems:
- NPI (National Provider Identifier) — Required by HIPAA, assigned by CMS through the NPPES
- Type 1 NPI — Individual providers (physicians, nurses, therapists)
- Type 2 NPI — Organizational providers (hospitals, group practices, labs)
- Provider taxonomy codes — Standardized classification codes that identify provider specialty and type
- CMS Certification Number (CCN) — Used to identify providers participating in Medicare/Medicaid
- DEA Number — For providers authorized to prescribe controlled substances
